임상적 치관이 너무 짧은 경우, 연조직에 문제를 일으킬 수 있고, 이와 동반하여 치주염으로 인한 치조골의 손상이 야기될 수 있기에, 장기적 안정성을 위해 생물학적 폭경과 적절한 임상치관의 길이가 확보되어야 한다. 그림 4 에서, 적절한 임상치관을 위한 공간이 부여되지 않은 경우, 구강 위생 유지 관리가 어려운 환경이 조성되어, 심한 임플란트 변연골 파괴를 동반한 임플란트 주위염 증례를 보여주고 있다(그림 4).

치아가 상실된 경우 적절한 시기에 보철치료를 하지 않을 경우 대합되지 않는 치아들의 정출로 인해 수직공간의 소실이 있을 수 있으므로, 임플란트 보철 치료를 위해 요구되는 최소한의 공간을 확보하기 위한 추가적인 치료가 필요할 것이다. 이 후의 내용에서 정출 정도에 따른 각각의 치료방법 및 고려사항에 대해 논의해 보고자 한다.
